All pricing below reflects self-pay rates.
Please note: We require a credit card on file for all patients.
Please read the policy section and the consent upon booking regarding refunds, contacting your provider, no-shows, and cancelations.
If you cancel less than 24 hours before your appointment, miss your appointment, or are more than 10 minutes late, you are responsible for the full self-pay rate, regardless of insurance status.
Psychiatric Evaluation
$250 | 60 minutes
A psychiatric evaluation is a 60-minute appointment where we take time to explore your history, concerns, sleep, mood, medications, and other important areas of your life.
During this visit, we may discuss potential diagnoses, treatment options including medications and supplements, and offer personalized recommendations. We provide evaluations for adolescents and adults.
Follow-up Visit
$125 | 30 minutes
Follow-ups are 30-minute appointments in which we can discuss how you are feeling, medication adjustments or changes, review labs, or even just check in to see if you're doing okay!
Same Day Refills
$10
We ask for 72 business hours to complete refill requests. If you run out of pills and need an urgent same-day request, there is a $10 processing fee.
Paperwork & Records
$15+
We can complete paperwork at your provider's discretion for employment, accommodations, educational concerns, emotional support animal letters, surgeries, and FMLA.
A simple document starts at $15. We charge an additional $25 per 15 minutes to complete complex paperwork.
All paperwork requires 72 business hours to complete. If you would like the form same day, there is a $100 fee for expediting.
